HEARTLinks Community Outreach Program
HEARTLinks is a community-based cardiovascular health outreach program designed to reduce barriers to care and improve heart health outcomes for underserved residents in Hillsborough and Pasco Counties. Through mobile health events and culturally responsive education, HEARTLinks connects individuals to free screenings, transportation support, and pathways to ongoing medical care.

The program brings free cardiovascular risk screenings directly into the community, increasing access to preventive services for individuals who may otherwise face financial, geographic, or systemic barriers. Participants also receive heart health education that is culturally responsive and tailored to community needs, helping to improve awareness of cardiovascular risk factors and support informed health decisions.
To address transportation challenges that often prevent follow-up care, HEARTLinks provides transportation assistance through ride-share options and volunteer drivers. Participants with abnormal screening results are supported through referrals to primary care providers or cardiology specialists, with follow-up efforts focused on promoting continuity of care.
HEARTLinks emphasizes meaningful community engagement through accessible outreach events, participant feedback, and ongoing relationship-building. Program effectiveness is evaluated using a combination of screening documentation, surveys, referral tracking, and qualitative feedback to ensure alignment with program goals and continuous improvement.
An annual program impact report is produced and shared with partners and stakeholders, summarizing participation, outcomes, and community feedback to support transparency and accountability.
